You may need to complete a supplementary information form (SIF) if you wish to apply for a school on faith or religious grounds. Religious criteria will be detailed in the oversubscription criteria of that school's individual admissions policy. Sometimes the SIF itself will be available to download from the policy document.
Some schools will also require other documentation (e.g. copy of baptism certificate). The SIF and any evidence must be submitted in addition to the main school application, and must also be submitted before the application deadline.
